Chronic Care Australia provides healthcare solutions for all Australians suffering from mental and physical chronic conditions. It developed and validated an exercise medicine delivery and prescription system that successfully treats, manages and prevents mental and physical illnesses concurrently.

In 2021 Chronic Care Australia received the Primary Care Award at the Value Based Health Care Awards which celebrates initiatives that adopt a new line of thinking in patient care, including outcomes and real costs. In December, it was a finalist in the Micro Business of the Year category at the 2021 Business News RISE Business Awards.
