In 2021 Chronic Care Australia received the Primary Care Award at the Value Based Health Care Awards which celebrates initiatives that adopt a new line of thinking in patient care, including outcomes and real costs. In December, it was a finalist in the Micro Business of the Year category at the 2021 Business News RISE Business Awards.
