There's the direct cost: the wages paid for underperformance, the overtime other staff pick up to cover gaps, the eventual cost of managing an exit and starting the search over again, this time under even more pressure than the first attempt. But the indirect cost is usually larger, and almost always invisible on a spreadsheet. A poor hire drags down the people working alongside them. Good staff start covering for a colleague who isn't pulling their weight, resentment builds quietly, and some of the business's best people start looking elsewhere, not because of pay or conditions, but because nobody dealt with the obvious problem sitting in the next bay.
By the time any of this becomes visible, it's rarely filed under "recruitment." It shows up as a training issue, a management issue, a culture issue. Someone gets sent on a leadership course. A new performance review template gets rolled out. What the Fix Actually Looks Like
The fix isn't a longer hiring process for its own sake. It's the same process, done with the steps that were cut back in.
It’s a proper skills assessment before an offer goes out, not after someone's already three weeks into the role and struggling. It’s a real reference conversation, one that asks about how someone handles pressure and works with a team, not just whether they turned up on time.
There needs to be a cultural fit check that treats "will this person actually work well with this specific team" as seriously as "can this person technically do the job," because a technically capable hire who clashes with the team around them can do more damage than an empty seat ever would.
None of this takes significantly longer than a rushed hire does. It just means doing the steps in the right order, with someone whose job is to catch the shortcut before it gets taken, rather than explain the fallout six months later.

Go back to that supervisor, incident report in hand, still trying to work out what went wrong with a hire that looked perfectly reasonable on paper. The honest answer isn't that she failed to manage someone properly. It's that the decision that mattered was made months earlier, in four rushed days, by people who never had to deal with what came after it.
A bad hire is never really a bad hire. It's a hiring process that skipped a step under pressure, and then waited politely for six months before sending the bill.
